A person over the age of 18 or their parents may request a copy of their birth certificate by either coming into Village Hall or by sending a letter or the attached form. If the person comes into Village Hall, the hours are Monday through Friday from 8:00 a.m. to 3:30 p.m. When writing in, the person needs to give us their name, date of birth, mother's maiden name and father's name, a photocopy of their driver's license (or another form of identification, i.e., non-driver's license, passport, naturalization papers, military ID, employer's photo ID, two utility bills, showing applicant's name and address, or a police report of lost or stolen ID) and a check or money order, made payable to Village of Suffern, for $10.00 per copy. It should then be mailed to Village of Suffern, Att: Vital Records, 61 Washington Avenue, Suffern, New York 10901. • BCFORM.pdf Death Certificates If a person passes away at Good Samaritan Hospital or within the Village of Suffern, a death certificate is on file at Village Hall. A relative of the deceased may request a copy of the death certificate by either coming into Village Hall or by sending a letter or the attached form. If the person comes into Village Hall, the hours are Monday through Friday from 8:00 a.m. to 3:30 p.m. When writing in, the person needs to give us the name of the deceased, their date of death, their relationship to the deceased and how many copies they need with or without the cause of death on the certificate and a check or money order, made payable to Village of Suffern, for $10.00 per copy. It should then be mailed to Village of Suffern, Att: Vital Records, 61 Washington Avenue, Suffern, New York 10901.
